Halcourt's COO had commissioned a full audit of the company's active automation environment following a failed ERP integration. The ERP failure had prompted the question nobody had asked before: what else is running that we do not understand? The audit findings were significant. Of the 23 distinct workflows identified, eleven had been built under a previous operations director and were not understood by any current staff member. When the audit team asked what each automation was measuring, and whether it had ever been formally reviewed against the outcome it was supposed to produce, the answers were consistent: there was no review mechanism. The four workflows that had documented success criteria at deployment had never been evaluated against them. Meridian's board reporting process touched six source systems: their ERP, their CRM, a project management platform, a separate billing system, a headcount tracker maintained in a shared spreadsheet, and an external benchmarking database updated quarterly. None of these systems spoke to each other. The CFO's senior analyst owned the assembly process -- a sequence of exports, manual consolidation in Excel, and reformatting that consumed her attention for the better part of the first week of every month. The discrepancy problem was the most urgent issue. Twice a quarter on average, a reconciliation error was discovered during assembly that had already been distributed in a preliminary version. The compliance officer sent individual requests, waited for responses, followed up when departments were late, and assembled the evidence package manually. Attestation submissions were consistently late. When the CEO was presented with the final package for signature, the cover summary indicated which requirements had been satisfied -- but there was no systematic way to verify that the underlying evidence was complete, current, and correctly matched to each requirement. The CEO was signing on trust, not on a validated record. The risk exposure this created had not been formally assessed until the compliance officer raised it directly.

Arbor's executive team reviewed KPI dashboards from three business units at their monthly leadership meeting. The dashboards were formatted differently, updated on different cadences, and used different definitions for the metrics they nominally shared. Gross margin was calculated differently across all three units -- one included freight, one did not, one allocated overhead by a method the other two did not use. Utilization was defined differently by each unit's operations leader, reflecting different approaches inherited from their original management teams. On-time delivery had three distinct definitions of what "on time" meant. The executive team had adapted to this by mentally discounting the numbers and relying on qualitative judgment. When the holding company's board began asking for consolidated performance metrics ahead of a potential acquisition, Arbor's CFO discovered that a meaningful consolidation was not possible without first resolving the definitional inconsistencies. The dashboard consolidation project that followed took six weeks. Five of those weeks were spent in alignment conversations before a single line of integration code was written.
